Appendix C: Notification Checklist for Executive Director, HQ Operations

Within 24-hours of mishap

NASA Administrator

Call or

e-mail

Occupational injury/illness of civil service or on-site contractor employee that resulted in:

•  A fatality, or

•  A permanent total disability, or

•  The hospitalization of in-patient care of 3 or more people within 30 workdays of the mishap, or

•  Property damage of $2 million or more.

Within 24-hours of mishap

NASA Administrator

Call or

e-mail

Occupational injury and/or illness of a civil service or on-site contractor employee that resulted in:

•  A permanent partial disability, or

•  The hospitalization for in-patient care of 1-2 people within 30 workdays of the mishap, or

•  Property damage of at least $500,000, but less than $2 million.

Within 24 hours

NASA Administrator

Call or

e-mail

Any on-site non-occupational fatality of NASA civil servant or contractor who works at or near HQ.

When notified by OIC or otherwise becomes known

NASA Administrator

Call or

e-mail

Any off-the-job fatality or serious injury/illness of a NASA civil servant or contractor who works at or near HQ.

Within 24 hours

Associate Administrator and Associate Administrator for Mission Support Directorate

Call or

e-mail

Non-fatal occupational injury/illness that resulted in any of the following beyond the day of mishap:

•  days away from work

•  restricted duty

•  transfer to another job.
