Appendix H: Example of Investigating Authority Appointment Letter

SUBJECT: Investigation of Type XX Mishap Short Title, IRIS no.

[Mishap Description]

 

 

[Mishap Report Due Date] The investigating authority will begin its investigation on ______________ and complete its investigation, providing an out brief and final report on or before __________(The investigator/team/board's completed investigation and report is expected within 60 calendar days).

[Mishap Team Charter] This memorandum establishes the investigating authority in accordance with NPR 8621.1, NASA Procedural Requirements for Mishap Reporting, Investigating, and Recordkeeping, to perform the investigation and determine the proximate and root causes of the mishap and make recommendations to minimize the probability of its recurrence. (Name) will chair the team, with (Names) serving as members.

The investigating authority will:

- Conduct activities in accordance with the requirements in NPR 8621.1.

- Designate any additional representatives, advisors, consultants, experts, liaison officers, or other individuals who may be required to support the activities of the investigation and define the duties and responsibilities of those persons.

I will dismiss the investigating authority when it has fulfilled its requirements.
