CHAPTER 1. Mishap and Close Call Reporting, Investigating, and Recordkeeping

1.1 Overview

1.1.1 NASA policy is to report, investigate, and document NASA mishaps, close calls, and previously unidentified serious workplace hazards to prevent recurrence and determine the root cause(s) in order to develop and implement corrective actions, in accordance with NPD 8621.1, "NASA Mishap and Close Call Reporting, Investigating and Recordkeeping." This HQPR establishes the HQ requirements for reporting, investigating, and documenting mishaps, close calls, and previously unidentified serious workplace hazards, in accordance with NASA and Occupational Safety and Health Administration (OSHA) requirements and policies, to prevent similar incidents recurring at HQ, and prevent injury, illness, and loss or damage to HQ property.

1.1.2 The HQPR does not apply to investigations involving civil, criminal, or administrative culpability or legal liability. The safety investigative process shall not be used to direct or justify disciplinary action.

1.1.3 The types of mishaps used in this HQPR are defined in Appendix A: Terms and Definitions.

1.2 Policy

1.2.1 It is HQ policy that all incidents i.e., mishaps and close calls shall be promptly reported to the responsible supervisor and the HQ Safety Office. Mishaps and close calls shall be investigated in order to identify cause(s), develop and implement corrective actions to prevent recurrence, and share lessons learned. Incidents that must be reported include: personnel injury and illness, fire, explosion, natural disaster, equipment or test failure, vehicle accident, environmental or other incidents, and close calls.
