PREFACE

P.1 PURPOSE

a. This HQ Procedural Requirements (HQPR) document describes the responsibilities and requirements for reporting, investigating, and documenting Headquarters (HQ) mishaps, close calls, and previously-unidentified serious workplace hazards.

b. The HQPR also serves as the HQ Mishap Preparedness and Contingency Plan to provide the procedures that must be taken for the immediate actions in the first hours following a mishap, the reporting of a mishap, and the investigation of the mishap process to closure.

c. The HQPR may be used with HQ emergency preparedness plans, including the HQ Continuity of Operations (COOP) plan, should a mishap impact the ability to continue operations at HQ.

P.2 APPLICABILITY

This HQPR applies to all NASA HQ personnel, operational and facilities activities, including contractors and their activities to the extent specified in their contracts.
