Appendix C. Form 1699B, Reasonable Accommodation Medical Evaluation Form

C.1 The process of requesting and receiving a reasonable accommodation decision is anticipated to be completed within 30 calendar days, however time spent to obtain and provide medical documentation (if necessary) is not counted toward the 30 calendar day timeframe.

C.2 In situations where the employee's disability is obvious to the Agency (e.g., the person has a hearing or visual impairment, or is in a wheel chair), there is generally no need to obtain medical documentation.

C.3 While an employee who seeks a particular accommodation but is granted an alternative effective accommodation is considered to have had his/her request approved in part, an employee may still seek reconsideration/appeal of that decision.

C.4 The process of obtaining and reviewing new medical documentation may add time to the reconsideration/appeal process.

C.5 In cases where no new information is provided to support the original reasonable accommodation request, the employee can waive the request for reconsideration by the 1st level supervisor, and immediately proceed to an appeal by the 2nd level supervisor. In these cases, the employee should inform the DPM who will contact the 2nd level supervisor to begin the appeal process.
