BENEFITS, NASA EMPLOYEE BENEFITS ASSOCIATION (NEBA)

SERVICE: The NEBA is an employee-administered organization that obtains low-cost group life insurance for NASA permanent employees; travel and accident insurance are offered. The insurance is underwritten by AH&L Health Company.

PROCEDURE: Permanent NASA employees should complete the enrollment form and return it to the Headquarters Human Resources Management, Code CP. You may obtain basic life insurance without submitting a health statement if you apply within 31 days after your NASA employment commences.

BONDS (GOVERNMENT SAVINGS)

SERVICE: Provides for the purchase of United States Series EE Savings Bonds by payroll deductions.

PROCEDURE: Fill in the information requested on Standard Form 1192, "U.S. Savings Bonds Authorization for Purchase and Request for Change," and submit to the Payroll Office.

BUILDING ALTERATIONS

SERVICE: Provides services for partition changes, electrical changes, painting, and related work.

PROCEDURE: Code contacts will submit NHQ Form 6, "Request for Facilities Management Services," with appropriate justification requesting building alterations. A Space Planning or Facility Manager representative will then contact requestor for details. Approval by necessary code contact is required.

BUILDING UTILITIES

SERVICE: Provides services for maintaining building utilities such as ceiling light replacements, electric power outages, restroom plumbing malfunctions, temperature adjustments, carpet cleaning and repairs, etc.

PROCEDURE: Telephone the problem to the Help Desk, Headquarters Facilities Management and Security Branch for corrective action.
