FARE SUBSIDY

SERVICE: Provides NASA Headquarters employees with subsidized transportation fare media for commuting.

PROCEDURE: Submit a completed and signed NHQ Form 201, "Application for Transit Benefit," to Code CP. Applicant must have been a NASA Headquarters employee for at least one month in order to be eligible. If applicant is a member of a carpool, or is assigned an individual parking permit, an NHQ Form 202, "Request for Change in Commuting Status," must be completed and returned to Code CP before the application can be approved.

Fare subsidy schedule is as follows: Tuesdays, Wednesdays, and Thursdays, 10 am. - 12 noon and 1:30-3 p.m., Room 6G73. NOTE: The last two days of distribution will only be half days 10 a.m. - 12 noon. Fare subsidies will be forfeited if not picked up by the last day. See HQMI 9730.1. "Fare Subsidies for NASA Headquarters Employees Utilizing Public Mass Transportation."

FITNESS FACILITY/STRESS LAB

SERVICE: Provides supervised exercise programs and rehabilitative therapy for NASA civil service employees. Priority is given to employees with health problems for which a fitness program has been prescribed. Aerobic, cardiovascular and muscle strengthening programs as well as health enhancement programs are offered. The facility includes various types of exercise equipment, locker rooms with showers, and dry saunas.

PROCEDURE: The NASA Headquarters Health Unit physician must medically approve each participant. Once medical clearance is received, the fitness staff will provide orientation, fitness assessments and customized workouts as appropriate.

FOREIGN TRAVEL

SERVICE: Provides required Foreign Travel Briefings to all employees planning official or personal travel to foreign countries or areas.

PROCEDURE: Employees assigned to National Security positions must notify their Code's Security Representative in writing, at least 3 weeks prior to the departure date, of all official or personal travel to foreign countries or areas. Once this information has been submitted to the Headquarters Facilities Management and Security Branch, a Foreign Travel Briefing will be given to the employee which addresses precautions that should be taken while traveling and visiting foreign countries or areas.

FORMS

SERVICE: Provides management analysis, design, and control of forms sponsored or used by Headquarters and arranges for them to be stocked for distribution.

PROCEDURE: To request development of a new form, revision or reprinting of an existing form, see contact listed below. Advice and assistance in managing electronic forms are also available from this office. Send order for bulk supplies of blank printed forms to the GSFC, Distribution Section, Code 239 on a completed NASA Form 2, "Request for Blank Forms, Publications and Issuances."

FORMS AND ISSUANCES (SUPPLY)

SERVICE: Provides Headquarters Offices with forms and management issuances.

PROCEDURE: Persons needing forms or copies of issuances may check for local availability through the GSFC Forms Catalog. Requestors may obtain items from the GSFC Forms Catalog by forwarding a NASA Form 2, "Request for Blank Forms, Publications and Issuances," to the office listed below. To order revisions to existing forms or create new forms, submit your request on NHQ Form 231 to the Records and Forms Manager, Code CFS, Headquarters, (202) 358-0621.
